Admission:
 
   Application for the 2010 - 2011 school year at Our Lady of Perpetual Help School has begun.  You may pick up the Application Packet at the School Office or in the forms tab at this web site.   This packet, along with all required documents, is to be returned to the Main Office when you apply.  Registration will take place only after all paperwork is complete, a student visit (see below) has taken place, an interview and testing have occurred, and your child has been granted admission.
 

 

Requirements for application are:

  • $25.00 application fee (Non-refundable)   

  • Birth Certificate

  • Baptismal Certificate (if Baptized in the Catholic Church)

  • Social Security Number

  • Grades One to Eight - Report Cards from current year and last two years as well as Standardized Reading and Math Scores

  • Kindergarten - Latest Report Card

  • Copy of any testing or evaluations conducted

  • Principal’s name, Teacher’s name, and Guidance Counselor, if applicable, with phone numbers and extensions

  • Letter of reference from current teacher or principal - when requested

  • A 2 day visit (consecutive days) for all children entering 2nd  through 8th Grade

  • A 1 day visit for all children entering 1st Grade

  • A ½ day visit (AM) for all children entering Kindergarten ½ day visit (PM) for all children entering Pre-K
 

Requirements for registration are:

  • Immunization Health Card

  • $100.00 Registration Fee (Non-refundable)   

  The minimum age requirements for registration are:

  • Pre-Kindergarten - four years old by December 31,2010

  • Kindergarten - five years old by December 31, 2010

  • First Grade - six years old by December 31, 2010
 

The hours of the school day for all children Kindergarten - Grade 8 are: 8:15 AM to 2:30 PM
   
  The hours of the school day for Pre- K children is:
   
AM session 8:15 - 12:00          Full Day 8:15-2:30  
   
 
 

After-school care will be available from 2:30-6 PM for an additional fee.
